And without question thanks to FJR for Metabar.

2. Press the "Install New Theme..." button select the downloaded theme.
3. Select the theme from your list of installed themes to make it current.
KDE 3.4.3 (and earlier):
1. Depending on how the theme was packaged, make a new directory for your new theme in ~/.kde/share/apps/metabar/themes.
2. Extract the theme to that directory.
3. Right-click on an active Metabar panel and select "Configure Metabar...".
4. Select the theme from your list of installed themes to make it current.

Update:
Just a minor change to the screen shots displayed.
1.5.1
There was a problem with the frame border and graphics widths not resizing well, sometime when the scrollbar came up the preview would be uncentered. I added a border to the frame in the css and reworked the graphics to be more "forgiving" to resizing the width. It can now be very narrow or wider and still look the way I intended.

I'm using SuSE 9.3 & KDE 3.5.1 updated via YaST2. After checking it looks like metabar provided by the package kdeaddons3-konqueror.
You might want to check this package. . .make sure it's installed correctly and such. Think that is where I'd start.
Hope that points you in the right direction.
