Book Template

I won't post a screenshot for this one. Out of pure frustration from talking to developers and searching the web for a easy book template for publishing I wiz-banged this one together just to get one done.
It has a (extremely) basic:
Cover Page - based on doc perfs
Table of Content - based on Header 3
Index - Based on content (req. manual build via indexing)
Its all in the OO.o help. thats how I built it by reading the help.
It would be great if someone built a gui driven template build.
